Register with us as a New Patient

We welcome new patients. Registration forms can be collected from Reception or downloaded from this website. We are happy to accept new patients irrespective of race, gender, ethnic origin, marital status, sexuality, existing medical condition(s) or social circumstances.

When you attend the Practice to register you will be asked to complete a new patient registration form, and new patient questionnaire form, these must be completed before you see a healthcare professional, so it is advisable to attend at least 10 minutes before your appointment time, so that you can compete the information required.

Practice Area

Our Practice Boundary covers the following postcode areas ML8, ML11 and ML12 when Woodstock Medical Centre is the closest surgery geographically.

Once you’ve been accepted as a patient, your medical records will be transferred to the Practice, there is no requirement for a medical card.

Your medical records may take several weeks to arrive at the Practice.

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.
