Integrated Community Support Service – Help you stay at Home

There are 4 Integrated Community Support Teams (ICSTs) in South Lanarkshire who provide adults with health & social care needs support, advice and rehabilitation within the home environment. Each team is made up of community nurses, physiotherapists, occupational therapists and support workers. The aim of the service is to support you to stay in your own home and avoid unnecessary hospital admissions.  We offer support, treatment and rehabilitation to help you to improve your health and support.
