The District Nursing Team is responsible for the assessment and nursing care of patients within their own homes. The District Staff facilitate early discharge from hospital by providing a high level of personal service according to individual needs. They also contribute to the community care of the chronic sick, terminally ill, elderly and housebound in the community.
The health visitors are qualified nurses with additional training in community health care. They are particularly skilled in seeking out and identifying health needs and work closely with all members of the Practice Team (GPs, Practice Nurses, District Nurses etc) to promote individual, family and public health. The health visitor works with all age groups, however, the majority of patients are families with pre-school children and the elderly.
